Appointments

We know you have many choices when choosing a doctor in Logan, UT, so we have made requesting an appointment a simple process via our Web site. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.

Payment Policies

We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details.

Services will never be denied strictly on the ability to pay. We do, however, request that patients who are paying cash to visit with us to discuss payment options. For elective surgeries we do require one half of our responsible amount be paid prior to surgery.

Insurance and Billing

We accept most traditional insurance plans. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.

It is our office policy to collect your insurance co-pay for services rendered in the office at the time of service. We will be happy to bill your insurance company for you, if you wish. We participate with most insurance plans, including Medicare and Medicaid.
